WHAT IS BEYOND HEALTH?
Beyond Health is a collaborative initiative to improve the community's health through a process of community health needs assessments and health improvement planning. The Community Health Assessment (CHA) is a vital opportunity to understand the health needs of our county. ln Brown County, the CHA is a product of cooperation between area public health agencies, health systems, community organizations, and community members. Every three years, the steering committee of Beyond Health brings together community stakeholders to review health data, provide knowledge and expertise, and identify health priorities.

Health priorities identified in the Community Health Assessment serve as the basis for the Community Health Improvement Plan (CHIP), which provides a framework and clear goals for community health initiatives over the following three years, including strategic work to be completed by relevant taskforces.

Both the CHA and the CHIP are activities which are grounded in national and state-level regulations. The Affordable Care Act, State of Wisconsin statutes and Public Health Accreditation Board (PHAB) clearly outline required steps for healthcare systems and accredited public health agencies to improve upon the health of communities. The Beyond Health steering committee provides the necessary support and oversight to these taskforces and ensures that collectively agreed upon goals are prioritized and met during each CHA/CHIP cycle.
