Brown County Community Health Assessment (posted June 10, 2026)

This survey is intended to be taken by people who live, work, play, or learn in Brown County. It was created by Beyond Health, which is a collaborative group including local health departments, health systems, and other health-focused organizations. Data from this survey will be used in the Community Health Assessment (CHA) and will help us decide which health priorities to focus on during out next Community Health Improvement Plan (CHIP) cycle.

You must be 18 years or older to take this survey. Brown County employees are not eligible for the gift card entry.
