A Community Health Assessment (CHA) is an effort to gather data to tell the story of health and identify public health probelms in a community. A CHA is typically done every three to five years. A Community Health Improvement Plan (CHIP) is done following a CHA. The purpose of the CHIP is to create a plan to address the public health problems identified in the CHA.
The Brown County CHA is a collaborative effort led by the Beyond Health Steering Committee.
